1 Reinstatement Information2 Athlete Information3 Reinstatement Details Amateur Reinstatement Information The request for reinstatement in to amateur competition is for athletes who have participated in professional combat sports competition, Muaythai competition in Thailand, or are presently licensed with an athletic commission in North America. Athletes who reinstate as amateurs are automatically considered to be A Class, however a request can be made to enter B Class or C Class based on a review of the athlete's abilities, experience, and past performance. To be eligible for placement in a less experienced class, the athlete must still fall within the experience requirements (0-3 contests for C Class, 4-9 contests for B Class) and may be required to provide supporting evidence of their professional experience. After completion, the form will be reviewed by a technical committee for approval. Athletes with professional experience in North America are subject to additional review and approval by the Athletic Commissioner of the province in which competition is taking place.I have read and understand the MTC Rules & Regulations, linked below*Rules & Regulations Yes I am looking to enter competition in the province of*(Please Select)ABBCMBNBNLNTNSNUONPEQCSKYTI am...*(Please Select)An athlete submitting my own reinstatement requestA coach/administator submitting on behalf of an athleteA parent/guardian submitting on behalf of my child Submitter's InformationYour Name* First Last Your Email* Enter Email Confirm Email Athlete InformationAthlete Legal Name* First Last Athlete Club Affiliation*Athlete Email* Enter Email Confirm Email Athlete Phone*Coach InformationCoach Name* First Last Coach Email Enter Email Confirm Email Coach Phone Professional Competition InformationInclude professional experience across all striking based combative sports.Have you ever won a professional title?*YesNoAre you currently under contract with any professional sports organizations or promoters?*YesNoAre you planning on participating in any professional contests in the next 12 months?*YesNoProfessional Wins*Professional Losses*Professional Draws*Professional Competition History*Failing to disclose your complete competition history will result in a rejected application.DateLocationSportSanction (MTO, MTC, TBA, etc)Weight (lbs)Opponent NameResult (Win/Loss/Draw/Demo) Amateur Competitive InformationInclude amateur experience across all striking based combative sports.Amateur Wins*Amateur Losses*Amateur Draws*Amateur Exhibitions/Demos/Smokers*Amateur Competition History*Failing to disclose your complete competition history will result in a rejected application.DateLocationSportSanction (MTO, MTC, TBA, etc)Weight (lbs)Opponent NameResult (Win/Loss/Draw/Demo) Reinstatement RequestDesired Experience Class for Reinstatement*Open Class (20+ Bouts)A Class (10+ Bouts)B Class (4-9 Bouts)C Class (0-3 Bouts)Supporting Video for Reinstatement Lower Than A Class*Please provide links to video footage of your professional competition.
